A vertically oriented, aged tan page is inked with black calligraphic script accented by small red marks. Seven lines of text top the page, followed by a blue heading above a grid of four small text boxes. Five additional lines conclude the bottom. Thin gold, red, and blue borders divide the sections. The fluid strokes of the Persian script contrast against the linear layout of the rectangular frames.
